The Blur adjustment applies uniformly to the entire image, not to a selected region. If you need to hide one specific detail, a practical alternative is to cover it with a solid shape using the Annotate tools, or crop it out entirely.
Once saved, the blur is baked into the exported file and the discarded detail cannot be recovered from that copy. Your original image remains unchanged on your device, so keep it if you may want the sharp version later.
The slider ranges from a barely perceptible softening to a heavy blur that reduces the image to washes of color. For background use, a moderate setting usually keeps enough structure to stay recognizable while letting foreground text pop.
